What is an AI Hallucination?

An AI hallucination occurs when the artificial intelligence generates inaccurate or fabricated information that appears plausible but is not based on the actual input data. In the context of NovoNote, this means the AI might create notes or summaries that include content not actually present in the transcript.

Data Security and Privacy Assurance

Rest assured that AI hallucinations do not involve accessing or sharing information from any other client; your privacy and confidentiality are fully maintained. Any details that appear in AI-generated notes that were not actually present in the session are created by the AI itself, based exclusively on the audio data from your session. NovoNote does not access any external data sources when generating these notes, nor has there been any compromise of your data security. 

These occurrences are a common part of AI generation and do not indicate a breach of privacy or data security. We encourage clinicians to review AI-generated content to ensure accuracy and confidently incorporate it into clinical records.

What Does AI Hallucination Look Like?

• Statements or details in the notes that do not match the actual conversation
• Information that seems plausible but is unsupported by the audio data. 
• Fabricated names, events, or facts not mentioned during the session

What Causes AI Hallucinations in NovoNote?

Several factors can contribute to hallucinations, including:

• Incomplete or Low-Quality Audio Transcription
  If parts of the conversation are unclear or not captured properly (for example, due to speech-to-text transcription errors), the AI may attempt to “fill in the gaps” by guessing or interpolating content.
  
  

• Limited Access to Full Content
  When the AI only has snippets of a meeting or is summarising in real-time without the full context, it may generate inaccurate or fabricated information based on partial data.

How to Minimise AI Hallucinations

To help ensure accurate notes, consider these best practices:

• Ensure the Microphone Captures Both Sides of the Conversation
  This is especially important for telehealth or remote sessions to capture the full dialogue clearly.
  
  

• Place the Microphone Close to the Audio Source in In-Person Sessions
  Positioning the microphone nearer to the speaker improves audio quality and transcription accuracy.
  
  

• Use High-Quality Recording Equipment and Minimise Surrounding Noise
  Clear audio reduces transcription errors that may confuse the AI.
  
  

• Review Notes for Accuracy
  Use AI as a collaborative assistant, and ensure accuracy by reviewing notes to ensure the final record accurately reflects the session.
